Request an Appointment
Appointments can be requested online. Please review the following instructions, and then click on the calendar below to request an open appointment time.
General
All appointment requests are subject to approval before they are confirmed and added to the calendar.
Appointment request fees will be applied to the final invoice due at the time of service, or refunded at request in the case of an overpayment or timely cancellation.
Choosing a visit type
Patients who have not yet been seen at our current location must choose a “new patient” opening.
Requests for treatment appointments will include a consult, if the consult for that treatment plan has not already occurred.
To be eligible for a telephone or video appointment, a patient must meet the following criteria:
Must have been seen by Jason Hone for an in-person annual exam within the last 12 months.
Must be present in the state of Utah at the time of the appointment
